Obverse: Coat of arms of Johannesburg with antelope supporters and lion crest with indistinct motto.

Reverse: Mine shaft headgear against the skyline of Johannesburg. In an arc above: “JOHANNESBURG”, and below: “1896 - 1986”

Notes: Issued inside a blue case lined on the inside with white silk marked: "SAM" and blue velvet.

The example of the silver medal shown here has a rectangular silver plaque inside the case: “PRESENTED TO / OOR HANDIG AAN / DR J.S. OTTO (engraved) / COMMEMORATION OF JOHANNESBURG’S CENTENARY / TER HERDENKING VAN JOHANNESBURG SE EEUFEES”. Dr Otto was Mayor of Johannesburg between 1978 and 1979.

Johannesburg is the largest city in South Africa and is is the provincial capital of Gauteng. The main Witwatersrand gold reef was discovered in June 1884 and Johannesburg was founded in 1886.
