Edge: Plain.

Obverse: Beaded border round the rim. On an ornate rectangular plaque across: "SOUTH AFRICA 1900". Behind, laurel wreath tied at the bottom with a ribbon, crossed bayoneted rifles with crown at the top. Stamped: "12" and ".5" at the foot.

Reverse: Within a laurel wreath tied at the bottom with a ribbon, space for engraving. Stamped at the top: "COLLINS" and at the bottom inside the wreath: "DGC" and the Birmingham hallmarks for 12ct (.5) gold and letter: "b" for 1901-2. This medal engraved: "SUTTON SURREY (in an arc) / 1901 / S. Chart".

Notes: The obverse is taken from the reverse of a series of medals of British generals (Laidlaw 0513a, 0513b and 0513c) by James Fenton.

The medal was presented to 409 Private S. Chart of the City of London Imperial Volunteers. He was awarded the Queen's South Africa Medal with four clasps for Johannesburg, Diamond Hill, Cape Colony and Orange Free State.
