Obverse: Coat of arms, rampant lion of the Cape of Good Hop on shield with motto: "TRIA JUNCTA IN UNO" on a band around. Round the rim on a band, Legend above: "GREY INSTITUTE" and below between stops: "PORT ELIZABETH CAPE OF GOOD HOPE".

Reverse: Blank band around the rim. Within, in an arc above surrounding space for engraving: "PRAEMIUM BIENNALE". This medal engraved: "1868 / HARRY JOHNSON". Below that across: "PUERO OPTIME MERITO / CON DISCIPULORUM / DUCI".

Notes: Sir George Grey (1812-1898) was Governor of Cape Colony between 1854 and 1861 and played a leading role in the establishment of educational institutes throughout the Colony. The Port Elizabeth municipality receive a land grant from the government some of which was sold to raise capital for the building of a school. The Grey Institute high school was formally opened in April 1859. The school today has over 800 boys and is now called Grey High School.

The school motto, Tria Juncta in Uno (three joined in one) is taken from the British Most Honourable Order of the Bath of which Sir George Grey was created a Knight Commander in 1848.
